Assistant Director

Service Coordination Program

 

Immediate opening for a new full time position to assist the Director of the Service Coordination Program in the management of the Luella Hannan Memorial Foundation's (Hannan) service coordination and social work program and practice. The Service Coordination Program coordinates the provision of services and programs as well as providing case management and other social work related services that enable individuals age sixty and older to live independently in senior housing and the community. This management position will involve supervision, project management, planning and evaluation.

Specific Duties

Subject to the specific authority and responsibility delegated by the Director of the Service Coordination Program, the following are examples of the duties required of this position:

  1. Supervision of Hannan employees or contractors who are service coordinators, social workers or otherwise staff Hannan's Service Coordination Program.
  2. Oversight of that portion of the work of any Hannan employee or contractor that is directly related to Hannan's Service Coordination Program.
  3. Provide staff support for Hannan's Strategic Alliance with Presbyterian Villages of Michigan and other special projects related to service coordination and social work in which Hannan is engaged.
  4. Planning and evaluation related to service coordination and social work practice.
  5. Field placement supervision of social work students assigned to Hannan's Service Coordination Program.
  6. Other duties as assigned by the Director of Service Coordination and Social Work Practice.

 

Job Qualifications

1. Master of Social Work degree and licensure

2. At least three years experience working with older adults

3. Experience in supervision and management

4. Ability to work as part of a team and independently

5. Planning and program development skills

6. Good oral and written communication skills

Service Coordinator for Senior Co-ops

Multiple Positions

 

The Service Coordinator will be responsible for overseeing and providing service coordination and case management for the residents of Co-ops, senior residences sponsored and managed by CSI Support & Development Services. The Service Coordinator's responsibility is to enhance the ability for members to stay living independently in the Co-op, and includes enhancing the work of the Family and Community Resource Committee (FCRC) and Education Committee, assessing needs and linking residents and community members to services, providing case management, planning and implementing educational opportunities promoting health and well-being, and developing and supporting meaningful engagement between building residents and residents of the surrounding community. The Service Coordinator will be an employee of the Luella Hannan Memorial Foundation and part of a joint Hannan-Co-op Team.

 

Job Duties and Responsibilities include:

  1. Working collaboratively with the Family and Community Resource Committee (FCRC) and Education Committee to enhance their current member-driven duties within the Co-op.
  2. Coordinating as well as conducting complete psychosocial assessments for residents on a voluntary basis who may be at risk or frail on an initial and ongoing basis.
  3. Helping transition applicants for residency at Co-op by determining their current and potential needs.
  4. Coordinating and/or providing general case management and referral services.
  5. Maintaining accurate, complete, updated files (both computer and hard files) on clients, programs and activities.
  6. Participating in regular meetings with the Hannan-Co-op Team and providing appropriate status reports.
  7. Establishing relationships with local service providers who are currently providing or could provide services to residents who are at risk or frail and monitoring the quality and effectiveness of their service delivery.
  8. Assuring that all service providers are "vetted" based on established criteria prior to making referrals.
  9. Working with the Hannan-Co-op Team to plan and implement educational and group programs.
  10. Assisting members to create and maintain informal support networks consisting of other members, family, and friends in their building(s) and the community.
  11. Supervising BSW and/or MSW and other interns as appropriate.
  12. Attending all appropriate Hannan Foundation and Co-op meetings.
  13. Continuously improve the quality of resident and community services that are provided by participating in regular training, research and evaluation of outcomes.

 

Job Qualifications:

1. Bachelor's degree in social work or related subject. LMSWs encouraged to apply.

2. Two-three+ years experience working with older adults in a community- based setting required.

3. Must have knowledge of senior community and area resources.

4. Demonstrated ability to work independently to achieve desired outcomes with older adults

individually and in groups to fulfill the requirements of the HUD service coordinator program.

5. Good oral and written and computer skills required. Must be able to accurately record and maintain

resident information and documentation.

6. Ability to work cooperatively with Foundation and Co-op Committees, members, and staff and to be flexible and creative in seeking solutions and solving problems.

7. Ability to create and maintain a positive atmosphere of customer service, including continuous

quality improvement, teamwork, mutual respect and professionalism.
